Click “next” to by-pass the insurance information. Under the tab “History” please fill out the COVID screening form and continue clicking “next” until the end of the forms. You do not need to fill out all the forms if you are an existing patient. Please fill out the COVID form honestly and accurately. Your wellbeing is important to us.
